What is the Radiance Dental Care Plan?

The plan is a way for us to thank you for your loyalty and to show how passionate we are about preventive, restorative, and advanced cosmetic dentistry. We know that preventive care is vital for good dental health and regular visits are important. This plan is especially significant for families who may have no insurance. The limitations inherent in most dental insurance plans such as deductibles, waiting periods, and annual maximums are not present with the Radiance Dental Care Plan.

What is the Enrollment Fee?

What is Included in the Care Plan?

For each person paying the annual enrollment fee, that individual is entitled to receive:

•	Two prophylactic dental cleanings per 12-month enrollment period.
•	Two dental exams during your cleaning appointments per 12-month period.
•	Any x-rays deemed necessary by the dentist in the 12-month period.
•	Application of flouride twice in the 12-month period for children under 16.
•	A significant savings for restorative dentistry, including fillings, crowns, extractions, root canals, dentures, periodontal treatment, veneers, teeth whitening, and dental implant restorations during the 12-month period.

Radiance Dental Care Plan membership provides significant cost savings on routine visits, peace of mind for unexpected emergencies, and most importantly, quality dental care in a comfortable environment. One of the main reasons that people end up needing major dental work is because they put off necessary dental care. When you come in for a regular cleanings and check-ups, we can help you avoid many of the more extensive and expensive dental procedures.

What is the Plan Duration?

The plan lasts 12 consecutive months from date of enrollment. All eligible treatment must be completed in that year period. The period begins with the date the enrollment fee is received.

What Services Are Included Every 12 Months?

•	Two Preventive Dental Cleanings
•	Two Exams performed by the Dentist
•	Digital X-Rays
•	Two Flouride Treatments for patients under age 16
•	Cosmetic Dentistry (Whitening and Veneers) 10% off the Usual and Customary Fee
•	Extractions 20% off the Usual and Customary Fee
•	Crowns and Bridges 20% off the Usual Customary Fee
•	Restorative fillings 20% off the Usual Customary Fee
•	Root Canal Therapy 20% off the Usual Customary Fee
•	New Partials and Full Dentures at 10% off the Usual Customary Fee
•	Repairs to Existing Partials and Full dentures are at the Usual Customary Fee

What are the Limitations and Exclusions?

•	Patients who are currently covered under a dental insurance plan are not eligible for membership. (This is because of the contractual restrictions placed on you and us by the insurance plan.) Membership fees do not apply for treatment originating from a Workers Compensation or Employer Liability Claim.
•	Going thirty (30) days beyond your individually prescribed re-care interval will void your Radiance Dental Care Plan membership with no refund of fees.
•	Joining of the plan cannot be done retroactively. Stated another way, you cannot join and then expect any rebate/refund on fees already paid.
•	Enrollment fees are non-refundable once any treatment has started.
•	If we determine we need to refer you to a specialist, the Radiance Dental Care Plan does not apply to the treatment received from the specialist.
•	If you become eligible and begin participation with a traditional dental insurance plan during the time period, the Radiance Dental Care Plan becomes null and void with no refund of fees.
•	“Family members” are defined as individuals living in the same residence or children under the age of 19.
•	The Care Plan fee is due in full upon joining. When applying the reduced fee schedule toward your treatment and services, payment is expected at the time of service, or the Radiance Dental Care Plan becomes null and void with no refund of fees.
•	It is the sole responsibility of the member to maximize their benefits by arranging all appropriate appointments within the 12-month care plan period.
•	If Radiance Dental Care Plan benefits are not used, the member will not be entitled to a refund.
•	Renewal payment is due at the same time each year, and benefits will run on a plan-year.
•	The annual renewal of your Radiance Dental Care Plan is exactly 12 months from the day you originally paid your annual plan fee.
•	Cancellation of two (2) dental appointments within 12 months without 48 hours’ notice will void your Radiance Dental Care Plan membership with no refund of fees.
